As we reach puberty our skin reaches the size it will be for the rest of our lives. However the underlying facial structures have a different story. The facial bones loose volume, our facial muscle mass decreases and the subcutaneous or fat layer also decreases in thickness. Since our skin doesn’t shrink and the other structures do, we see generalized loosening of the skin over the aging facial frame with further sagging, worsened by the natural forces of gravity. This results in decreased fullness of the cheek, sagging of the fat pad, which helps support the cheek. The nasolabial folds thicken and become more prominent. Of course gravity also contributes to these changes to further change the lower face with increased wrinkles around the mouth, and down turning of the corners of the mouth. Commonly there is formation of marionette lines which are folds that occur from the corner of the mouth to the chin. Another common problem seen is formation of the ever famous jowls.
